Cremasteric reflex


Test for the cremaster reflex. Used in patients with suspected cauda equina syndrome, in multiple sclerosis, testicular torsion, upper and lower motor neuron lesions, lesions at the level of L1/L2. The reflex also tests for the integrity of the ilioinguinal (sensory) nerve or the genitofemoral nerve (motor).

The cremasteric reflex is a superficial (i.e., close to the skin’s surface) reflex observed in human males.

This reflex is elicited by lightly stroking or poking the superior and medial (inner) part of the thigh regardless of the direction of stroke. The normal response is an immediate contraction of the cremaster muscle that pulls up the testis ipsilaterally (on the same side of the body). The reflex utilizes sensory and motor fibers from two different nerves. When the inner thigh is stroked, sensory fibers of the ilioinguinal nerve are stimulated. These activate the motor fibers of the genital branch of the genitofemoral nerve which causes the cremaster muscle to contract and elevate the testis.

In boys, this reflex may be exaggerated which can occasionally lead to a misdiagnosis of cryptorchidism.

The cremasteric reflex may be absent with testicular torsion, upper and lower motor neuron disorders, as well as a spine injury of L1L2. It can also occur if the ilioinguinal nerve has accidentally been cut during a hernia repair.

The cremasteric reflex can be helpful in recognizing testicular emergencies. The presence of the cremasteric reflex does not eliminate testicular torsion from a differential diagnosis, but it does broaden the possibilities to include epididymitis or other causes of scrotal and testicular pain. In any event, if testicular torsion cannot be definitively eliminated in an expeditious manner, a testicular Doppler ultrasound or exploratory surgical intervention is usually implemented to prevent possible loss of the testicle to necrosis.

The deep tendon reflex (DTR) examination is part of the neurologic exam. They were first described by Wilhelm Heinrich Erb and Carl Friedrich Otto Westphal more than a century ago. Their use continues to this day. The presence of hyporeflexia or hyperreflexia may indicate an underlying disease. Proper technique and interpretation of results are crucial to help in the diagnosis of many upper and lower motor neuron pathologic processes such as multiple sclerosis, amyotrophic lateral sclerosis, spinal cord injuries, spinal muscular atrophies, among others. They are sometimes referred to as muscle stretch reflexes.

A reflex arc is an involuntary pathway by which the stimulus to a tendon elicits a muscle response. It is a monosynaptic reflex where only two neurons are involved; a sensory neuron provides the afferent component, and a motor neuron at the spinal cord provides the efferent component. The pathway initiates in the muscle spindle, a proprioceptive organ. The muscle spindle is composed of intrafusal fibers, engulfed by a connective tissue capsule responsible for detecting muscle stretch. The muscle spindle is present within the muscle in between the extrafusal fibers.
