• As the cecum becomes filled and distends, a local reflex causes:
• Closure of the ileocecal valve.
• Activation of haustral churning.
• Haustral churning mixes the chyme, which helps absorption of water, salts, and vitamins.
• Haustral churning propels the contents of the colon along the large intestine.
• The gastrocolic reflex is also triggered when food enters and distends the stomach.
• This reflex intensifies strong mass peristalsis movements that force feces into the rectum
• The defecation reflex is activated by stretch receptors stimulated by the filling of the rectum.
• Defecation is an ANS reflex in which sensory impulses are sent to the control center in the spinal cord.
• Parasympathetic impulses travel down the spinal sacral nerves and:
• Stimulate longitudinal contractions of the rectum forcing feces into the anal canal.
• Relax the internal anal sphincter allowing the feces to move toward the external anal sphincter.
• Voluntary relaxation of the external anal sphincter allows defecation to be completed.

The gastrocolic reflex or gastrocolic response is one of a number of physiological reflexes controlling the motility, or peristalsis, of the gastrointestinal tract. It involves an increase in motility of the colon in response to stretch in the stomach and byproducts of digestion in the small intestine. Thus, this reflex is responsible for the urge to defecate following a meal. The small intestine also shows a similar motility response. The gastrocolic reflex helps make room for more food.
The reflex was demonstrated by myoelectric recordings in the colons of animals and humans, which showed an increase in electrical activity within as little as 15 minutes after eating. The recordings also demonstrated that the gastrocolic reflex is uneven in its distribution throughout the colon. The sigmoid colon is more greatly affected than the right side of the colon in terms of a phasic response; however, the tonic response across the colon is uncertain. When pressure within the rectum becomes increased, the gastrocolic reflex acts as a stimulus for defecation. A number of neuropeptides have been proposed as mediators of the gastrocolic reflex. These include serotonin, neurotensin, cholecystokinin (CCK), and gastrin.
Clinically, the gastrocolic reflex has been implicated in pathogenesis of irritable bowel syndrome: the very act of eating or drinking can provoke an overreaction of the gastrocolic response in some patients with irritable bowel syndrome due to their heightened visceral sensitivity, and this can lead to abdominal pain, diarrhea, or constipation. Also, the serotonin (5HT3) antagonist ondansetron decreases the tonic response to stretch.

This hasn’t happened to me in a while, but this was an almost everyday occurrence during my childhood. Halfway through a meal, I would cramp up, rush to the bathroom, hunch over in tears, and sit there for as long as it took to feel better (if I was so lucky). Obviously, things are somewhat easier now after 23 years of practicing deep breathing and relaxation techniques, but that doesn’t make these episodes any less draining or inconvenient.
The emotional part of symptoms like these for anyone with IBS, IBD, or any other GI malfunction is having a wonderful day interrupted by something you own yet have no control over. The pain might be mild on good days, but then the thought sticks, “Is this the beginning of something worse or was that it?” I may be wrong with this term, but it’s almost like triggering PTSD: that one cramp, regardless of severity, reminds you of all the horrific scenarios already overcome and, unfortunately, likely to happen again.
Staying busy is one of my best techniques for not falling down that rabbit hole because it leaves no time to contemplate, no time to ponder.
